Important

Before you submit

Is this a quote or enrollment?

No. This is only an interest form. Nothing is quoted, enrolled, bound, or guaranteed.

Why are agencies being counted together?

A larger agency group may create more options than each agency shopping alone. The goal is to evaluate whether enough agencies want to review a pooled risk approach designed for more stable long-term renewals.

Do you ask health questions?

No. Do not submit health conditions, claims, medications, diagnoses, or other health information.

Should I go to the marketplace instead?

The marketplace may be a fallback for some individuals, but it is usually not built around an agency group, employer contribution strategy, or long-term pooled renewal stability. If you want your agency considered for this group review, get counted here before defaulting to an individual-market path. Keep current coverage in force until replacement coverage is confirmed in writing.
